Es reconeix per un dolor sobtat i intens, com si alg\u00fa t\u2019hagu\u00e9s colpejat per darrere. El temps de recuperaci\u00f3 oscil\u00b7la entre 2 setmanes per a lesions lleus i 8 setmanes o m\u00e9s en ruptures greus.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Lo_essencial_dun_cop_dull\"><\/span><strong>Lo essencial d\u2019un cop d\u2019ull:<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">&#8211; El s\u00edmptoma m\u00e9s caracter\u00edstic \u00e9s una sensaci\u00f3 brusca de &#8220;cop de pedra&#8221; al panxell.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8211; Existeixen tres graus de gravetat, i cadascun exigeix un abordatge diferent.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8211; La <a href=\"https:\/\/www.mgc.es\/ca\/clinicas\/epsilon-physio\/\">fisioter\u00e0pia<\/a> \u00e9s el pilar del tractament; tornar abans d\u2019hora multiplica el risc de recaiguda.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8211; L\u2019ecografia musculoesquel\u00e8tica \u00e9s la prova que confirma el diagn\u00f2stic i el grau de la lesi\u00f3.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Que_es_un_esquincament_muscular_al_besso_o_%E2%80%9Csindrome_del_cop_de_pedra%E2%80%9D\"><\/span><strong>Qu\u00e8 \u00e9s un esquin\u00e7ament muscular al bess\u00f3 o &#8220;s\u00edndrome del cop de pedra&#8221;?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">El m\u00fascul gastrocnemi, conegut popularment com a bess\u00f3, forma la major part del volum del panxell. Juntament amb el soli, compon el tr\u00edceps sural, el conjunt muscular que ens permet impulsar-nos en caminar, c\u00f3rrer o saltar. Quan la tensi\u00f3 generada durant un moviment supera la capacitat el\u00e0stica de les seves fibres, aquestes es trenquen, total o parcialment. Aix\u00f2 \u00e9s un esquin\u00e7ament muscular o ruptura fibril\u00b7lar.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La lesi\u00f3 afecta amb m\u00e9s freq\u00fc\u00e8ncia el cap intern del bess\u00f3, que \u00e9s la porci\u00f3 m\u00e9s medial i la que suporta m\u00e9s c\u00e0rrega en els canvis de ritme. Per aix\u00f2 apareix tant en esportistes amateurs que practiquen tennis, p\u00e0del o running com en persones que fan un esfor\u00e7 puntual sense preparaci\u00f3 pr\u00e8via.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>La sensaci\u00f3 de &#8220;cop de pedra&#8221;: per qu\u00e8 es produeix aquest dolor sobtat<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">El nom popular ve de l\u2019experi\u00e8ncia que descriu gaireb\u00e9 tothom qui el pateix: la sensaci\u00f3 de rebre un cop fort al panxell, seguida de dolor immediat i dificultat per recolzar el peu. El que passa en aquell instant \u00e9s una contracci\u00f3 brusca del m\u00fascul que supera el seu l\u00edmit el\u00e0stic. Les fibres s\u2019esquincen de manera sobtada, cosa que activa els receptors del dolor instant\u00e0niament i explica aquesta percepci\u00f3 d\u2019impacte extern que, en realitat, ve de dins.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Graus de la ruptura fibril\u00b7lar: I, II i III<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">No totes les ruptures s\u00f3n iguals. La classificaci\u00f3 habitual distingeix tres graus:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Grau I (microruptura):<\/strong> es trenquen poques fibres musculars. El dolor \u00e9s moderat, la funci\u00f3 es conserva en bona mesura i la recuperaci\u00f3 se sol completar en 1-2 setmanes.<\/li>\n\n\n\n<li><strong>Grau II (ruptura parcial):<\/strong> la lesi\u00f3 afecta un nombre significatiu de fibres, per\u00f2 el m\u00fascul no perd continu\u00eftat. Apareixen hematoma, inflamaci\u00f3 notable i impot\u00e8ncia funcional parcial. La recuperaci\u00f3 requereix entre 3 i 6 setmanes.<\/li>\n\n\n\n<li><strong>Grau III (ruptura completa):<\/strong> el m\u00fascul es trenca en tota la seva secci\u00f3 transversal. El dolor \u00e9s intens, la impot\u00e8ncia funcional \u00e9s total i, en alguns casos, pot requerir intervenci\u00f3 quir\u00fargica. La recuperaci\u00f3 supera les 8 setmanes.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Simptomes_per_identificar_un_esquincament_al_besso\"><\/span><strong>S\u00edmptomes per identificar un esquin\u00e7ament al bess\u00f3<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"880\" height=\"586\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/si\u0301ntomas-para-identificar-un-desgarro-en-el-gemelo-880x586.jpg\" alt=\"\" class=\"wp-image-39770\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/si\u0301ntomas-para-identificar-un-desgarro-en-el-gemelo-880x586.jpg 880w, https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/si\u0301ntomas-para-identificar-un-desgarro-en-el-gemelo-440x293.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/si\u0301ntomas-para-identificar-un-desgarro-en-el-gemelo-768x511.jpg 768w, https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/si\u0301ntomas-para-identificar-un-desgarro-en-el-gemelo-scaled.jpg 1024w\" sizes=\"auto, (max-width: 880px) 100vw, 880px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Distingir un esquin\u00e7ament d\u2019una contractura o d\u2019una cosa m\u00e9s seriosa, com una trombosi venosa profunda, \u00e9s important per actuar b\u00e9 des del primer moment. La clau est\u00e0 en com apareix el dolor i en els signes que l\u2019acompanyen.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Una <a href=\"https:\/\/www.mgc.es\/ca\/blog\/massatges-i-estiraments-com-podem-alleujar-les-contractures-musculars\/\">contractura muscular se sol instal\u00b7lar de manera progressiva<\/a>, amb tensi\u00f3 i mol\u00e8stia creixent. L\u2019esquin\u00e7ament, en canvi, t\u00e9 un inici brusc i molt localitzat. Si el dolor va apar\u00e8ixer de cop durant l\u2019esfor\u00e7 i no pots recolzar el peu amb normalitat, les probabilitats que sigui una ruptura fibril\u00b7lar s\u00f3n altes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La trombosi venosa profunda es pot confondre perqu\u00e8 tamb\u00e9 produeix dolor i inflor al panxell, per\u00f2 en aquest cas el dolor no sol estar relacionat amb un esfor\u00e7 concret i pot anar acompanyat de calor intensa i envermelliment de la pell. Davant d\u2019aquesta combinaci\u00f3 de s\u00edmptomes, ves a urg\u00e8ncies sense esperar.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Dolor agut, espetec i impot\u00e8ncia funcional immediata<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">En el moment exacte de la lesi\u00f3, els s\u00edmptomes s\u00f3n molt espec\u00edfics. El dolor \u00e9s agut, puntual i apareix de cop. Algunes persones expliquen haver sentit un espetec o crepitaci\u00f3 al panxell. Immediatament despr\u00e9s, la cama &#8220;no respon&#8221; amb normalitat: recolzar el peu fa mal, posar-se de puntetes resulta impossible o molt dolor\u00f3s i caminar es torna dif\u00edcil.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Aparici\u00f3 d\u2019hematoma i inflor en les hores seg\u00fcents<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les primeres 24-48 hores porten nous signes. La ruptura de fibres danya tamb\u00e9 petits vasos sanguinis, cosa que provoca una equimosi (blau) que pot apar\u00e8ixer a la zona de la lesi\u00f3 o baixar cap al turmell per efecte de la gravetat. La inflamaci\u00f3 (edema) genera una sensaci\u00f3 de tensi\u00f3 al panxell. Tots dos signes confirmen el dany tissular i s\u00f3n indicadors \u00fatils per a l\u2019especialista a l\u2019hora de valorar la gravetat.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Temps_de_recuperacio_de_lesquincament_muscular_al_besso_fase_a_fase\"><\/span><strong>Temps de recuperaci\u00f3 de l\u2019esquin\u00e7ament muscular al bess\u00f3: fase a fase<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Aquest \u00e9s el punt que m\u00e9s preocupa: quant triga a curar? La resposta dep\u00e8n del grau de la lesi\u00f3, per\u00f2 el proc\u00e9s sempre segueix les mateixes fases.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Fase 1 (Aguda, 24 a 72 hores): rep\u00f2s i m\u00e8tode RICE<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les primeres hores s\u00f3n decisives per limitar el dany secundari. El protocol cl\u00e0ssic \u00e9s el RICE: <strong>R<\/strong>ep\u00f2s, <strong>I<\/strong>ce (gel), <strong>C<\/strong>ompressi\u00f3 i <strong>E<\/strong>levaci\u00f3 (en angl\u00e8s). A la pr\u00e0ctica: recolza la cama com menys millor, aplica gel embolicat en un drap durant 15-20 minuts diverses vegades al dia, fes servir un embenat compressiu suau i mant\u00e9n la cama elevada sempre que puguis. L\u2019objectiu \u00e9s reduir el sagnat intern i la inflamaci\u00f3. En aquesta fase, la calor est\u00e0 completament contraindicada.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Fase 2 (Subaguda, setmana 1 a 3): inici de la fisioter\u00e0pia i mobilitat controlada<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Passades les primeres 72 hores, el fisioterapeuta entra en escena. El treball en aquesta fase busca afavorir la cicatritzaci\u00f3 del teixit i recuperar la mobilitat sense for\u00e7ar les fibres en reparaci\u00f3. Les t\u00e8cniques habituals inclouen massoter\u00e0pia transversa profunda, drenatge limf\u00e0tic per reduir l\u2019edema, electroter\u00e0pia i exercicis de mobilitat passiva i activa sense c\u00e0rrega. Caminar amb suport progressiu sol ser possible cap al final d\u2019aquesta fase en lesions de grau I o II lleu. No saltar-se aquest pas, encara que el dolor hagi cedit, \u00e9s una de les decisions m\u00e9s importants de tota la recuperaci\u00f3.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Fase 3 (Readaptaci\u00f3, setmana 3 a 6 o m\u00e9s): enfortiment i tornada a l\u2019activitat<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Quan el teixit cicatricial t\u00e9 prou resist\u00e8ncia, comen\u00e7a la fase m\u00e9s exigent. S\u2019introdueixen exercicis d\u2019enfortiment progressiu del tr\u00edceps sural, amb especial atenci\u00f3 al treball exc\u00e8ntric (descens lent del tal\u00f3 en un esgla\u00f3, per exemple). La propiocepci\u00f3, \u00e9s a dir, l\u2019entrenament de l\u2019equilibri i la coordinaci\u00f3 neuromuscular, \u00e9s clau per reduir el risc de recaiguda. La tornada a l\u2019esport es fa de manera esglaonada: primer cursa suau en l\u00ednia recta, despr\u00e9s canvis de ritme, i finalment gestos t\u00e8cnics espec\u00edfics de l\u2019esport practicat.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Factors que acceleren o retarden la recuperaci\u00f3<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">El grau de la lesi\u00f3 \u00e9s el factor m\u00e9s determinant, per\u00f2 no l\u2019\u00fanic. L\u2019edat, l\u2019estat f\u00edsic previ, la rapidesa amb qu\u00e8 es va iniciar el tractament i, sobretot, el compliment de les pautes del fisioterapeuta marquen difer\u00e8ncies reals en els terminis finals. Una ruptura de grau I tractada b\u00e9 des del primer dia es pot resoldre en 10 dies; la mateixa lesi\u00f3 ignorada es pot cronificar.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Diagnostic_i_tractament_que_esperar_a_la_consulta\"><\/span><strong>Diagn\u00f2stic i tractament: qu\u00e8 esperar a la consulta<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"880\" height=\"862\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/diagno\u0301stico-y-tratamiento-que\u0301-esperar-en-la-consulta-880x862.jpg\" alt=\"\" class=\"wp-image-39771\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/diagno\u0301stico-y-tratamiento-que\u0301-esperar-en-la-consulta-880x862.jpg 880w, https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/diagno\u0301stico-y-tratamiento-que\u0301-esperar-en-la-consulta-440x431.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/diagno\u0301stico-y-tratamiento-que\u0301-esperar-en-la-consulta-768x753.jpg 768w, https:\/\/www.mgc.es\/wp-content\/uploads\/2026\/06\/diagno\u0301stico-y-tratamiento-que\u0301-esperar-en-la-consulta-scaled.jpg 1024w\" sizes=\"auto, (max-width: 880px) 100vw, 880px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Quan vas al traumat\u00f2leg o al metge esportiu, l\u2019exploraci\u00f3 f\u00edsica sol ser suficient per sospitar el diagn\u00f2stic. L\u2019especialista palpar\u00e0 la zona dolorosa, valorar\u00e0 la mobilitat del turmell i et demanar\u00e0 que facis certs moviments per avaluar la funci\u00f3 muscular.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>L\u2019ecografia com a prova clau per confirmar el grau de la lesi\u00f3<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019ecografia musculoesquel\u00e8tica \u00e9s la prova d\u2019imatge d\u2019elecci\u00f3 per als esquin\u00e7aments musculars. Permet visualitzar en temps real l\u2019extensi\u00f3 de la ruptura, l\u2019hematoma associat i l\u2019estat del teixit circumdant. \u00c9s r\u00e0pida, no emet radiaci\u00f3 i ofereix informaci\u00f3 din\u00e0mica que la <a href=\"https:\/\/www.mgc.es\/ca\/blog\/que-es-el-radiodiagnostic\/\">resson\u00e0ncia magn\u00e8tica<\/a>, encara que m\u00e9s detallada, no sempre aporta en el context d\u2019urg\u00e8ncia. El resultat guia directament les decisions de tractament.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Errors comuns durant la recuperaci\u00f3 que has d\u2019evitar<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Alguns errors es repeteixen molt sovint i allarguen innecess\u00e0riament la lesi\u00f3:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Aplicar calor en les primeres 48-72 hores.<\/strong> La calor augmenta la vasodilataci\u00f3 i agreuja el sagnat intern. Nom\u00e9s s\u2019introdueix en fases posteriors i sota criteri del fisioterapeuta.<\/li>\n\n\n\n<li><strong>Tornar a l\u2019activitat quan el dolor cedeix.<\/strong> L\u2019abs\u00e8ncia de dolor no equival al fet que el teixit estigui curat. La cicatriu muscular triga setmanes a adquirir resist\u00e8ncia funcional real.<\/li>\n\n\n\n<li><strong>Saltar-se la fisioter\u00e0pia.<\/strong> El rep\u00f2s sol no regenera el teixit de manera \u00f2ptima.
