En aquest cas, cal mesurar la quantitat de contingut mineral \u00f2ssia mitjan\u00e7ant una <a href=\"https:\/\/www.cun.es\/enfermedades-tratamientos\/pruebas-diagnosticas\/densitometria-osea\">densitometria<\/a>.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"880\" height=\"586\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/06\/Imagen1-880x586.jpg\" alt=\"\" class=\"wp-image-35444\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/06\/Imagen1-880x586.jpg 880w, https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/06\/Imagen1-440x293.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/06\/Imagen1-768x511.jpg 768w, https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/06\/Imagen1-1536x1023.jpg 1536w, https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/06\/Imagen1-scaled.jpg 1024w\" sizes=\"auto, (max-width: 880px) 100vw, 880px\" \/><\/figure>\n\n\n\n<p>La freq\u00fc\u00e8ncia de la malaltia augmenta amb l&#8217;edat, poques vegades apareix abans dels 55 anys i les dones es veuen afectades amb m\u00e9s freq\u00fc\u00e8ncia que els homes. Les fractures de pacients d&#8217;osteoporosi es produeixen fonamentalment a la columna vertebral a l&#8217;altura de l&#8217;esquena, als canells i al coll del f\u00e8mur (l&#8217;os de la cama en l&#8217;entroncament amb el maluc). Cada any es produeixen a Espanya 33.000 casos de <a href=\"https:\/\/www.msdmanuals.com\/es\/hogar\/traumatismos-y-envenenamientos\/fracturas\/fracturas-de-cadera?ruleredirectid=756\">fractura de maluc<\/a> a causa d&#8217;osteoporosi. Com veurem, la malaltia es pot prevenir amb una vida sana, una dieta rica en calci i suplements de <a href=\"https:\/\/www.mayoclinic.org\/es\/drugs-supplements-vitamin-d\/art-20363792\">vitamina D<\/a> i en concret per a les dones, mitjan\u00e7ant un tractament hormonal substitutiu amb estr\u00f2gens despr\u00e9s de la menopausa. At\u00e8s que gaireb\u00e9 sempre les fractures m\u00e9s perilloses s\u00f3n conseq\u00fc\u00e8ncia d&#8217;una caiguda, la prevenci\u00f3 de caigudes \u00e9s de vital import\u00e0ncia.<\/p>\n\n\n\n<p>L&#8217;osteoporosi \u00e9s una malaltia cr\u00f2nica, per\u00f2 hi ha medicaci\u00f3 per augmentar el contingut en calci dels ossos i disminuir el risc de noves fractures.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Per_que_apareix\"><\/span><strong>Per qu\u00e8 apareix<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Es desconeix perqu\u00e8 la p\u00e8rdua espont\u00e0nia de massa \u00f2ssia, que comen\u00e7a als trenta anys en algunes persones, \u00e9s m\u00e9s intensa que en altres, i produeix osteoporosi. Pot haver-hi factors hereditaris o ser el resultat de no haver format prou os a la joventut, ser causada per una vida massa sedent\u00e0ria o per un esc\u00e0s consum de calci a la dieta.<\/p>\n\n\n\n<p>S \u201cassocien amb l&#8217;osteoporosi:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>La menopausa preco\u00e7 (abans dels 45 anys)<\/li>\n\n\n\n<li>El tabaquisme<\/li>\n\n\n\n<li>El consum d&#8217;alcohol<\/li>\n\n\n\n<li>Algunes malalties cr\u00f2niques (artritis reumatoide, asma, malalties intestinals i <a href=\"https:\/\/www.msdmanuals.com\/es\/hogar\/trastornos-hormonales-y-metab%C3%B3licos\/trastornos-de-las-gl%C3%A1ndulas-suprarrenales\/s%C3%ADndrome-de-cushing?ruleredirectid=756\">s\u00edndrome de Cushing<\/a>).<\/li>\n<\/ul>\n\n\n\n<p>El tractament perllongat amb corticoides (prednisona, cortisona) pot augmentar el risc d&#8217;osteoporosi si la dosi \u00e9s superior a 7,5 mil\u00b7ligrams diaris. Malauradament, en el tractament d&#8217;algunes malalties cr\u00f2niques \u00e9s inevitable l&#8217;\u00fas de corticoides<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Quins_simptomes_produeix\"><\/span><strong>Quins s\u00edmptomes produeix<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Molts pacients presenten dolors ossis, sovint a l&#8217;esquena i natges, per\u00f2 pot no apar\u00e8ixer cap s\u00edmptoma fins que es produeix una fractura, ja que la disminuci\u00f3 de massa \u00f2ssia per si mateixa no produeix s\u00edmptomes. Ja hem comentat que les fractures m\u00e9s freq\u00fcents s\u00f3n les nines, les v\u00e8rtebres i els malucs. Els pacients amb aixafaments vertebrals presenten una <a href=\"https:\/\/medlineplus.gov\/spanish\/ency\/article\/001240.htm\">postura encorbada<\/a> (cifosi), amb disminuci\u00f3 de la talla corporal i les costelles poden arribar a contactar amb la pelvis, causant una disminuci\u00f3 de la capacitat tor\u00e0cica i dolor en respirar.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Quins_son_els_signes_dalarma\"><\/span><strong>Quins s\u00f3n els signes d&#8217;alarma<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>L&#8217;aparici\u00f3 de fractures espont\u00e0nies o per un traumatisme m\u00ednim, com ara una caiguda lleugera<\/li>\n\n\n\n<li>Un espetec a l&#8217;esquena (ocasionat a les v\u00e8rtebres) amb dolor fort, d&#8217;aparici\u00f3 brusca<\/li>\n\n\n\n<li>La menopausa primerenca<\/li>\n\n\n\n<li>El pes corporal baix, el tabaquisme i els antecedents familiars d&#8217;osteoporosi<\/li>\n\n\n\n<li>El tractament prolongat amb corticoides (com la prednisona) i altres medicaments que poden causar la disminuci\u00f3 de calci als ossos.<\/li>\n\n\n\n<li>La pres\u00e8ncia de malalties que es poden associar amb osteoporosi, com ara l&#8217;artritis reumatoide<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Com_es_pot_prevenir_losteoporosi\"><\/span><strong>Com es pot prevenir l&#8217;osteoporosi<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Fer exercici sistem\u00e0ticament \u2014durant tota la vida<\/li>\n\n\n\n<li>Seguir una dieta amb continguts adequats de calci i vitamina D.<\/li>\n<\/ul>\n\n\n\n<p>A la taula seg\u00fcent es resumeixen les necessitats di\u00e0ries de vitamina D i la ingesta \u00f2ptima de calci, segons van ser definides a la confer\u00e8ncia de consens de l&#8217;Institut Nacional de la Salut als Estats Units:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><tbody><tr><td><strong>Edat (anys)<\/strong><\/td><td><strong>Recomanacions de vitamina D (micrograms\/dia)<\/strong><\/td><td><strong>Recomanacions de calci (mil\u00b7ligrams\/dia)<\/strong><\/td><\/tr><tr><td>&lt;1\/2<\/td><td>10<\/td><td>400<\/td><\/tr><tr><td>&lt;1\/2-1<\/td><td>10<\/td><td>600<\/td><\/tr><tr><td>1-5<\/td><td>10<\/td><td>800<\/td><\/tr><tr><td>6-10<\/td><td>2,5<\/td><td>800-1200<\/td><\/tr><tr><td>11-24<\/td><td>2,5<\/td><td>1.200-1.500<\/td><\/tr><tr><td>Homes de 25 a 65<\/td><td>2,5<\/td><td>1.000<\/td><\/tr><tr><td>Dones de 25 a 50<\/td><td>2.5<\/td><td>1.000<\/td><\/tr><tr><td>Embarassades<\/td><td>10<\/td><td>1.200-1.500<\/td><\/tr><tr><td>Dones en lact\u00e0ncia<\/td><td>10<\/td><td>1.200<\/td><\/tr><tr><td>Homes &gt; 65<\/td><td>2.5<\/td><td>1.500<\/td><\/tr><tr><td>Dones &gt;50 amb THS (*)<\/td><td>2.5<\/td><td>1000<\/td><\/tr><tr><td>Dones &gt;50 sense THS<\/td><td>2.5<\/td><td>1.500<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>(*) TSH: Tractament Hormonal Substitutiu.<\/p>\n\n\n\n<p>Una regla senzilla per a adults \u00e9s intentar consumir 1.000 mil\u00b7ligrams de calci al dia (per exemple dos gots de llet i un iogurt o 100 grams de formatge). Despr\u00e9s de la menopausa, els experts en osteoporosi recomanen una dosi de 1.200 a 1.500 mil\u00b7ligrams diaris de calci. La mateixa quantitat deuen prendre els pacients que estan en tractament amb corticoides.<\/p>\n\n\n\n<p>Les persones a qui no els agrada la llet (o hi s\u00f3n al\u00b7l\u00e8rgics) han de prendre suplements de calci, alguns <a href=\"https:\/\/www.reumatologiaclinica.org\/es-suplementos-calcio-vitamina-d-para-articulo-S1699258X11001422\">en combinaci\u00f3 amb vitamina D<\/a>. La quantitat de calci del suplement dependr\u00e0 de la ingesta de productes lactis. Els suplements s&#8217;han de prendre combinats amb els \u00e0pats en 2 o 3 dosis di\u00e0ries.<\/p>\n\n\n\n<p>La vitamina D es troba en l&#8217;oli de peix, en conserves de peix com arengades, sardines, salm\u00f3 o verat i tamb\u00e9 es poden sintetitzar a la pell, amb la llum solar. A l&#8217;estiu, cal recomanar a les persones grans que prenguin el sol durant mitja o una hora al dia (cura amb les cremades!) i consumir peixos grassos amb regularitat (sardines, seitons, cavalles). Si no es du a terme aix\u00f2 es requerir\u00e0 un suplement de vitamina D de 10 micrograms (o 400 unitats).<\/p>\n\n\n\n<p>La majoria dels experts en osteoporosi recomana que els pacients que la pateixen prenguin 40 micrograms (600 unitats) de vitamina D al dia, a m\u00e9s de suplements de calci.<\/p>\n\n\n\n<p>Cal evitar fumar i beure alcohol. La p\u00e8rdua anual de massa \u00f2ssia \u00e9s el doble en fumadors que no fumadors. Es pot culpar el tabac d&#8217;un de cada sis trencaments de maluc. \u00c9s important adaptar la llar i modificar l&#8217;estil de vida per evitar caigudes, ja que una de cada vint caigudes a casa produeix fractures i, d\u201d aquestes, una de cada cinc \u00e9s de maluc.<\/p>\n\n\n\n<p>S&#8217;ha de considerar el <a href=\"https:\/\/www.elsevier.es\/es-revista-medicina-integral-63-articulo-terapia-hormonal-sustitutiva-posmenopausia-13036885\">tractament amb hormones<\/a> (estr\u00f2gens) a les dones, sobretot si han tingut menopausa preco\u00e7.<\/p>\n\n\n\n<p>\u00c9s benefici\u00f3s i altament recomanable fer exercici durant 30 minuts almenys tres cops per setmana.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Com_es_diagnostica_losteoporosi\"><\/span><strong>Com es diagnostica l&#8217;osteoporosi<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Quan es produeix una fractura amb un traumatisme m\u00ednim, la causa m\u00e9s freq\u00fcent \u00e9s l&#8217;osteoporosi i no sol ser necessari fer una densitometria. Si el pacient no ha patit fractures, s\u00ed que cal fer la prova (per mesurar la densitat mineral de l&#8217;os) i poder diagnosticar adequadament l&#8217;osteoporosi.<\/p>\n\n\n\n<p>Fins i tot en les ocasions en qu\u00e8 la densitometria no \u00e9s necess\u00e0ria (quan hi ha fractures pr\u00e8vies) s&#8217;ha de fer un examen f\u00edsic i una an\u00e0lisi de sang que permetin assegurar que es tracta d&#8217;osteoporosi, ja que hi ha altres malalties que tamb\u00e9 poden causar fractures i p\u00e8rdua de calci, els tractaments de les quals difereixen dels de l&#8217;osteoporosi. Per exemple, faltes de vitamina, diferents tipus de c\u00e0ncer o malalties de la medul\u00b7la \u00f2ssia poden causar fractures \u00f2ssies. Aix\u00ed i tot, l&#8217;osteoporosi \u00e9s la causa m\u00e9s freq\u00fcent de fractures.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"El_futur\"><\/span><strong>El futur<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Quan ja existeix una fractura causada per osteoporosi, el risc que el pacient en presenti una de nova \u00e9s m\u00e9s del doble que en persones sanes, encara que varia amb el contingut de calci als ossos.
