Per digerir-la, el cos utilitza una enzima anomenada lactasa, que es produeix en l\u2019intest\u00ed prim i descompon la lactosa, que \u00e9s un sucre complex, en els seus dos components simples: glucosa i galactosa.<\/p>\n\n\n\n<p>La intoler\u00e0ncia a la <a href=\"https:\/\/lactosa.org\/la-intolerancia\/primeras-preguntas\/\">lactosa<\/a> \u00e9s la incapacitat d\u2019algunes persones de digerir-la, generalment per la falta de lactasa o per poca producci\u00f3 d\u2019aquesta enzima. La lactosa sense digerir es despla\u00e7a sense processar intest\u00ed endavant i arriba al c\u00f2lon, on les bacteries \u201cataquen\u201d la lactosa i produeixen els s\u00edmptomes propis de la mancan\u00e7a.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"800\" height=\"488\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/02\/lacteos-variados.jpg\" alt=\"\" class=\"wp-image-33734\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/02\/lacteos-variados.jpg 800w, https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/02\/lacteos-variados-440x268.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2025\/02\/lacteos-variados-768x468.jpg 768w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Que_la_produeix\"><\/span><strong>Qu\u00e8 la produeix<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>L\u2019alimentaci\u00f3 dels nadons, a base de llet, requereix que l\u2019intest\u00ed segregui lactasa, per poder digerir-la. A mesura que el nen creix, i va substituint la llet per altres aliments, la producci\u00f3 de lactasa disminueix, per\u00f2 no s\u2019atura i gr\u00e0cies a aix\u00f2 en l\u2019edat adulta podem seguir ingerint productes l\u00e0ctics sense problemes. Podem parlar de <a href=\"https:\/\/www.clinicaremei.org\/es\/article\/intolerancia-lactosa-ninos\">tres tipus d\u2019intoler\u00e0ncia<\/a> a la lactosa:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Intoler\u00e0ncia prim\u00e0ria<\/strong>, que \u00e9s quan la producci\u00f3 de lactasa disminueix dr\u00e0sticament, dificultant la ingesti\u00f3 dels l\u00e0ctics. Es presenta en l\u2019edat adulta.<\/li>\n\n\n\n<li><strong>Intoler\u00e0ncia secund\u00e0ria<\/strong>, que es produeix com a conseq\u00fc\u00e8ncia d\u2019una lesi\u00f3 o una malaltia que afecti l\u2019intest\u00ed prim. Les malalties que poden potencialment afectar la producci\u00f3 de lactasa s\u00f3n la malaltia cel\u00edaca, la malaltia de Crohn i les infeccions intestinals en general.<\/li>\n\n\n\n<li><strong>Intoler\u00e0ncia cong\u00e8nita<\/strong>. Hi ha casos, poc freq\u00fcents, de nens que neixen amb insuficient producci\u00f3 de lactasa. \u00c9s un problema gen\u00e8tic i es transmet de generaci\u00f3 en generaci\u00f3, ambd\u00f3s progenitors han de portar el gen perqu\u00e8 es manifesti en els seus fills.<\/li>\n<\/ul>\n\n\n\n<p>At\u00e8s que les c\u00e8l\u00b7lules productores de la lactasa no apareixen, sin\u00f3 en el tercer trimestre de l\u2019embar\u00e0s, els nens prematurs poden presentar problemes d\u2019intoler\u00e0ncia.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Quins_son_els_seus_simptomes\"><\/span><strong>Quins s\u00f3n els seus s\u00edmptomes<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>En general, els s\u00edmptomes apareixen entre 30 minuts i 2 hores despr\u00e9s d\u2019haver ingerit els productes l\u00e0ctics, i els m\u00e9s habituals s\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Inflor abdominal<\/li>\n\n\n\n<li>Diarrea<\/li>\n\n\n\n<li>N\u00e0usees<\/li>\n\n\n\n<li>V\u00f2mits<\/li>\n\n\n\n<li>Flatul\u00e8ncia (gasos)<\/li>\n\n\n\n<li>C\u00f2lics abdominals<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Factors_de_risc_dintolerancia_a_la_lactosa\"><\/span><strong>Factors de risc d\u2019intoler\u00e0ncia a la lactosa<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Hi ha determinats factors que poden predisposar a patir intoler\u00e0ncia a la lactosa:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Origen<\/strong>: La intoler\u00e0ncia \u00e9s m\u00e9s freq\u00fcent entre persones africanes, asi\u00e0tiques, hispanes o natives americanes. A Espanya \u00e9s molt freq\u00fcent i s\u2019estima que la pateix entre un 30 i un 50% de la poblaci\u00f3.<\/li>\n\n\n\n<li><strong>Edat<\/strong>: En envellir, la producci\u00f3 de lactasa tendeix a disminuir, amb la qual cosa la intoler\u00e0ncia \u00e9s m\u00e9s freq\u00fcent.<\/li>\n\n\n\n<li><strong>Prematurs<\/strong>: Com ja vam dir, els mecanismes de producci\u00f3 de la lactasa no s\u2019engeguen fins al final de l\u2019embar\u00e0s, la qual cosa genera intoler\u00e0ncia en els nadons prematurs.<\/li>\n\n\n\n<li>Patir celiaquia, malaltia de Crohn, la s\u00edndrome de l\u2019intest\u00ed irritable, infeccions intestinals o haver sofert cirurgies o tractaments oncol\u00f2gics tamb\u00e9 predisposen a la intoler\u00e0ncia.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Com_es_diagnostica\"><\/span><strong>Com es diagnostica<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Hi ha diversos m\u00e8todes de diagn\u00f2stic:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Prova de l\u2019hidrogen espirat<\/strong>. Es subministra al pacient un l\u00edquid amb lactosa i es mesura la quantitat d\u2019hidrogen que expira.<\/li>\n\n\n\n<li>Com a alternativa o complement de l\u2019anterior, es pot realitzar una <strong>an\u00e0lisi de sang<\/strong> que mesuri la gluc\u00e8mia, o nivell de glucosa en sang abans i despr\u00e9s de la ingesta del preparat.<\/li>\n\n\n\n<li><strong>An\u00e0lisi de femta<\/strong>, per mesurar el pH. Si la lactosa no s\u2019absorbeix, augmenta l\u2019\u00e0cid l\u00e0ctic, la qual cosa faria disminuir el pH (\u00e0cid).<\/li>\n\n\n\n<li>En ocasions es recomana una dieta d\u2019una o dues setmanes lliures de lactosa per avaluar si es presenten s\u00edmptomes o no.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Tractament\"><\/span><strong>Tractament<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>La majoria dels pacients no necessiten cap tractament per al trastorn. En general, una certa cura en la ingesti\u00f3 de l\u00e0ctics sol ser suficient. Els l\u00e0ctics que millor es digereixen s\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Els l\u00e0ctics fermentats, com el iogurt<\/li>\n\n\n\n<li>Els formatges curats<\/li>\n\n\n\n<li>La mantega<\/li>\n<\/ul>\n\n\n\n<p>Els s\u00edmptomes solen desapar\u00e8ixer en eliminar els l\u00e0ctics de la dieta, per\u00f2 suprimir per complet els l\u00e0ctics pot tenir complicacions, com mancances de calci i vitamina D. El metge o el farmac\u00e8utic podran recomanar-te el m\u00e9s adequat.<\/p>\n\n\n\n<p>En el cas dels nadons i nens petits intolerants, ser\u00e0 el pediatre qui determini el tractament.<\/p>\n\n\n\n<p>Actualment existeixen molts productes lliures de lactosa en el mercat, des de llet sense lactosa fins a llet a la qual s\u2019afegeix lactasa.
