T\u00e9 un paper crucial en la producci\u00f3 dels gl\u00f2buls vermells i de l&#8217;ADN, i controla que el sistema nervi\u00f3s es mantingui sa. Tamb\u00e9 ajuda a prevenir el cansament i la fatiga i refor\u00e7a el sistema immunitari.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Lanemia_perniciosa_i_el_factor_intrinsec\"><\/span><strong>L&#8217;an\u00e8mia perniciosa i el factor intr\u00ednsec<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Quan mengem aliments rics en vitamina B<sub>12<\/sub>, la vitamina es combina amb una prote\u00efna anomenada factor intr\u00ednsec secretada a l&#8217;est\u00f3mac. La combinaci\u00f3 de totes dues (de la vitamina i la prote\u00efna) viatja per l&#8217;intest\u00ed prim, on l&#8217;organisme l&#8217;absorbeix.<\/p>\n\n\n\n<p>Quan un pacient t\u00e9 an\u00e8mia perniciosa, l&#8217;organisme produeix anticossos contra el factor intr\u00ednsec i les c\u00e8l\u00b7lules que el secreten. La vitamina no es pot combinar i, per tant, el cos \u00e9s incapa\u00e7 d&#8217;absorbir-la. No \u00e9s clar per quines raons apareix aquest trastorn.<\/p>\n\n\n\n<p>L&#8217;an\u00e8mia perniciosa sovinteja en dones m\u00e9s grans de 50 anys i si hi ha antecedents familiars. Tamb\u00e9 \u00e9s relativament freq\u00fcent en persones amb altres trastorns autoimmunitaris, com ara la malaltia d&#8217;Addison (una alteraci\u00f3 de les gl\u00e0ndules suprarenals), l&#8217;artritis reumatoide i certs problemes tiroidals.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Causes_de_la_manca_de_vitamina_B12\"><\/span><strong>Causes de la manca de vitamina B<sub>12<\/sub><\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Sovint se&#8217;n t\u00e9 manca, especialment en el cas de la gent gran. El risc augmenta significativament si no se n&#8217;ingereix prou amb la dieta, o si es t\u00e9 cap problema per absorbir-la.<\/p>\n\n\n\n<p>Ja hem vist que l&#8217;an\u00e8mia perniciosa \u00e9s tant causa com conseq\u00fc\u00e8ncia de la mala absorci\u00f3 de la vitamina B<sub>12<\/sub>. Altres causes habituals d&#8217;aquesta manca s\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>La medicaci\u00f3<\/strong><\/li><\/ul>\n\n\n\n<p>Hi ha medicaments que poden alentir i, fins i tot, blocar l&#8217;absorci\u00f3 de la vitamina B<sub>12<\/sub>. Els que es fan servir m\u00e9s sovint s\u00f3n els anti\u00e0cids inhibidors de la bomba de protons, com ara l&#8217;omeprazole, que funcionen reduint la secreci\u00f3 d&#8217;\u00e0cid a l&#8217;est\u00f3mac i s\u00f3n necessaris per alliberar la B<sub>12<\/sub> dels aliments; i la metformina, que \u00e9s el medicament m\u00e9s emprat per controlar la diabetis de tipus 2.<\/p>\n\n\n\n<p>Altres f\u00e0rmacs associats a l&#8217;absorci\u00f3 de la vitamina B<sub>12<\/sub> s\u00f3n uns quants tractaments antiepil\u00e8ptics, com la colquicina (que s&#8217;administra per tractar la gota) i l&#8217;antibi\u00f2tic neomicina. Les dones que prenen la p\u00edndola a llarg termini tamb\u00e9 poden estar fins a un cert punt en perill, si b\u00e9 el risc \u00e9s inferior que amb altres f\u00e0rmacs.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>La dieta<\/strong><\/li><\/ul>\n\n\n\n<p>Els seguidors estrictes de dietes veganes (aproximadament el 0,8 % de la poblaci\u00f3 espanyola) poden tenir nivells baixos de vitamina B<sub>12<\/sub>, per\u00f2 no \u00e9s gaire habitual en dietes ben equilibrades.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Els problemes d&#8217;est\u00f3mac<\/strong><\/li><\/ul>\n\n\n\n<p>\u00c9s freq\u00fcent en pacients que s&#8217;han sotm\u00e8s a intervencions que eliminen la part de l&#8217;intest\u00ed on s&#8217;absorbeix la vitamina, i tamb\u00e9 en persones amb problemes intestinals com ara la malaltia de Crohn o la gastritis atr\u00f2fica (en qu\u00e8 el teixit intestinal \u00e9s molt prim). Els pacients amb <em>Helicobacter pylori <\/em>tamb\u00e9 poden patir defici\u00e8ncies de vitamina B<sub>12<\/sub>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Simptomes_de_la_deficiencia_de_vitamina_B12\"><\/span><strong>S\u00edmptomes de la defici\u00e8ncia de vitamina B<sub>12<\/sub><\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>La manca de vitamina B<sub>12<\/sub> es manifesta amb s\u00edmptomes associats a l&#8217;an\u00e8mia i els m\u00e9s habituals s\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Pal\u00b7lidesa de la pell<\/li><li>Cansament i fatiga inexplicable<\/li><li>Sensaci\u00f3 de desmai<\/li><li>Mal de cap<\/li><li>Palpitacions<\/li><li>Mal humor<\/li><li>Ac\u00fafens (o tinnitus, que \u00e9s la percepci\u00f3 de sorolls inexistents)<\/li><\/ul>\n\n\n\n<p>A m\u00e9s a m\u00e9s, l&#8217;an\u00e8mia produeix \u00falceres bucals, adoloriment de la llengua i clivelles a les comissures de la boca. Tamb\u00e9 es poden sentir punxades i fiblades a les mans i als peus, tenir problemes de visi\u00f3, canvis d&#8217;humor i una tonalitat de la pell d&#8217;un groc llimona.<\/p>\n\n\n\n<p>La vitamina B<sub>12<\/sub> fa un paper important en la producci\u00f3 de mielina (la subst\u00e0ncia que recobreix els nervis). La defici\u00e8ncia de vitamina B<sub>12<\/sub> pot afectar aquesta funci\u00f3 i afavorir sensacions estranyes a les mans i als peus.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"800\" height=\"533\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/11\/sangreanalisis2.jpg\" alt=\"\" class=\"wp-image-27232\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/11\/sangreanalisis2.jpg 800w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/11\/sangreanalisis2-440x293.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/11\/sangreanalisis2-768x512.jpg 768w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Diagnostic_de_la_manca_de_vitamina_B12\"><\/span><strong>Diagn\u00f2stic de la manca de vitamina B<sub>12<\/sub><\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Es diagnostica amb facilitat amb una an\u00e0lisi de sang que mesura el nivell de la vitamina al torrent sanguini. Si el nivell \u00e9s baix, caldr\u00e0 fer m\u00e9s an\u00e0lisis per determinar el grau d&#8217;an\u00e8mia perniciosa, en cas que correspongui (hi ha tests per a anticossos de les c\u00e8l\u00b7lules del revestiment de l&#8217;est\u00f3mac i el factor intr\u00ednsec). Igualment, es podria fer la prova de Schilling, que fa servir una forma radioactiva de la vitamina B<sub>12<\/sub> per avaluar el factor intr\u00ednsec.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Tractament_contra_la_manca_de_vitamina_B12\"><\/span><strong>Tractament contra la manca de vitamina B<sub>12<\/sub><\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>El tractament habitual \u00e9s amb injeccions de vitamina B<sub>12<\/sub>; de bon comen\u00e7ament les punxades s\u00f3n freq\u00fcents per fer-ne cr\u00e9ixer les reserves del cos, per\u00f2, despr\u00e9s, se n&#8217;espaia la periodicitat a cada dos o tres mesos per mantenir ben cobertes les necessitats. Aquest tractament es pot allargar tota la vida, tret que la defici\u00e8ncia de vitamina B<sub>12<\/sub> sigui conseq\u00fc\u00e8ncia d&#8217;una dieta inadequada; aleshores, abans que optar per les injeccions, se&#8217;n mira de restablir els nivells amb la dieta.<\/p>\n\n\n\n<p>Les dosis recomanades en cas d&#8217;an\u00e8mia perniciosa depenen de si hi ha problemes nerviosos. En tot cas, el metge ha de determinar la freq\u00fc\u00e8ncia inicial i successiva de les injeccions.<\/p>\n\n\n\n<p>Per elevar la dosi de vitamina B<sub>12<\/sub> ingerida en la dieta, recomanem d&#8217;augmentar el consum de carn vermella, de peix i de mol\u00b7luscos, tamb\u00e9 de cereals enriquits, llet desnatada, iogurt, formatge i ous.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Seguiment_en_cas_de_falta_de_vitamina_B12\"><\/span><strong>Seguiment en cas de falta de vitamina B<sub>12<\/sub><\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>La majoria dels pacients que segueixen un tractament contra la manca de vitamina B<sub>12<\/sub> hi noten milloria r\u00e0pidament quan el comencen. Aconsellem de comprovar-ne els nivells amb una an\u00e0lisi de sang anual, que cal complementar amb un recompte de c\u00e8l\u00b7lules, un control dels nivells de folat i de la funci\u00f3 tiroidal (els pacients amb an\u00e8mia perniciosa tenen una mica m\u00e9s problemes de gl\u00e0ndules tiroides). Cal tenir en compte que, si es produeix cap dany en el sistema nervi\u00f3s com a conseq\u00fc\u00e8ncia de la defici\u00e8ncia de vitamina B<sub>12<\/sub>, podria ser dif\u00edcil de tractar i potser no ens en recuperar\u00edem mai del tot.<\/p>\n\n\n\n<p>L&#8217;an\u00e8mia perniciosa augmenta el risc de desenvolupar c\u00e0ncer d&#8217;est\u00f3mac: un pacient que en pateixi o que n&#8217;hagi patit \u00e9s fins a tres cops m\u00e9s procliu a tenir-ne, fins i tot si s&#8217;ha tractat l&#8217;an\u00e8mia.
