El sistema circulatori fa una funci\u00f3 cabdal, com \u00e9s dur a les c\u00e8l\u00b7lules l&#8217;aliment que necessiten (sobretot oxigen) i, alhora, eliminar els residus (com ara l&#8217;anh\u00eddrid carb\u00f2nic o el CO2) que les c\u00e8l\u00b7lules produeixen. I, per cert, sabies que la sang \u00e9s un teixit i no pas un simple l\u00edquid corporal?<\/p>\n\n\n\n<p><strong>Els gl\u00f2buls vermells<\/strong><\/p>\n\n\n\n<p>La sang t\u00e9 b\u00e0sicament quatre components: el plasma (que n&#8217;\u00e9s la part l\u00edquida), els gl\u00f2buls blancs (que serveixen per combatre les infeccions), les plaquetes (que s&#8217;enllacen entre si per cicatritzar les ferides) i els gl\u00f2buls vermells.<\/p>\n\n\n\n<p>Els gl\u00f2buls vermells (tamb\u00e9 anomenats hematies o b\u00e9 eritr\u00f2cits) s\u00f3n les c\u00e8l\u00b7lules encarregades de transportar oxigen. Tenen una mena de bosseta amb una prote\u00efna \u2014l&#8217;hemoglobina\u2014 que s&#8217;enlla\u00e7a f\u00e0cilment amb l&#8217;oxigen i el CO2.<\/p>\n\n\n\n<p>Es pateix an\u00e8mia quan el cos no \u00e9s capa\u00e7 de produir prou gl\u00f2buls vermells, o b\u00e9 en produeix de defectuosos.<\/p>\n\n\n\n<p><strong>S\u00edmptomes de l&#8217;an\u00e8mia i diagn\u00f2stic<\/strong><\/p>\n\n\n\n<p>Sigui quina sigui la classe d&#8217;an\u00e8mia, que descriurem tot seguit, els s\u00edmptomes m\u00e9s habituals s\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Manca d&#8217;energia i fatiga<\/li><li>Feblesa<\/li><li>Mareigs o sensaci\u00f3 de defalliment<\/li><li>Dificultat per respirar<\/li><li>Irritabilitat<\/li><li>Mals de cap<\/li><li>Dificultat per concentrar-se<\/li><li>Palpitacions o freq\u00fc\u00e8ncia card\u00edaca accelerada<\/li><li>Pal\u00b7lidesa<\/li><\/ul>\n\n\n\n<p>Si sospitem que patim an\u00e8mia, el metge demanar\u00e0 una an\u00e0lisi espec\u00edfica de la sang, en la qual es manifestar\u00e0 el d\u00e8ficit corresponent a la classe d&#8217;an\u00e8mia que t\u00e9 el pacient:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Nivells an\u00f2mals (m\u00e9s alts o baixos del compte) de gl\u00f2buls vermells o d&#8217;hemoglobina.<\/li><li>Nivells d&#8217;hemat\u00f2crits massa baixos (es mesura l&#8217;espai que ocupen els gl\u00f2buls vermells a la sang).<\/li><li>Volum corpuscular mitj\u00e0 (VCM), o mida mitjana dels gl\u00f2buls vermells.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"800\" height=\"533\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/naranjas2.jpg\" alt=\"\" class=\"wp-image-26561\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/naranjas2.jpg 800w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/naranjas2-440x293.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/naranjas2-768x512.jpg 768w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><figcaption>branch orange tree fruits green leaves in Valencia Spain<\/figcaption><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Classes_danemia\"><\/span><strong>Classes d&#8217;an\u00e8mia<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>B\u00e0sicament n&#8217;hi ha tres classes diferents: l&#8217;an\u00e8mia per manca d&#8217;\u00e0cid f\u00f2lic, l&#8217;an\u00e8mia per manca de vitamina B<sub>12<\/sub> i l&#8217;an\u00e8mia per manca de ferro. Tot seguit aprofundirem en cada patologia.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Anemia_per_manca_dacid_folic\"><\/span><strong>An\u00e8mia per manca d&#8217;\u00e0cid f\u00f2lic<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>L&#8217;\u00e0cid f\u00f2lic \u00e9s una vitamina que t\u00e9 un paper clau en la s\u00edntesi de l&#8217;ADN (material del nucli de les c\u00e8l\u00b7lules que cont\u00e9 els gens). Quan tenim manca d&#8217;\u00e0cid f\u00f2lic, es deteriora la capacitat de s\u00edntesi de l&#8217;ADN, fet que perjudica la formaci\u00f3 de c\u00e8l\u00b7lules noves, entre d&#8217;altres dels gl\u00f2buls vermells, que es fabriquen a la medul\u00b7la \u00f2ssia (dins dels ossos). La manca d&#8217;\u00e0cid f\u00f2lic fa minvar el nombre de gl\u00f2buls vermells a la sang, \u00e9s a dir causa an\u00e8mia.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Que causa aquesta an\u00e8mia<\/strong><\/li><\/ul>\n\n\n\n<p>La manca o d\u00e8ficit d&#8217;\u00e0cid f\u00f2lic \u00e9s causat, sobretot, per una nutrici\u00f3 defectuosa. Els aliments que contenen \u00e0cid f\u00f2lic s\u00f3n la fruita i les verdures; aix\u00ed doncs, cal que comprovis si en consumeixes prou. Quina quantitat? Doncs recorda la regla de cinc: cinc peces de fruita (o racions de verdura) al dia.<\/p>\n\n\n\n<p>Per contra, hi ha subst\u00e0ncies que perjudiquen l&#8217;absorci\u00f3 de l&#8217;\u00e0cid f\u00f2lic: l&#8217;alcohol principalment, per\u00f2 tamb\u00e9 uns quants medicaments, com ara la p\u00edndola anticonceptiva, o certs f\u00e0rmacs antiepil\u00e8ptics.<\/p>\n\n\n\n<p>Hi ha malalties, tamb\u00e9, que en disminueixen l&#8217;absorci\u00f3: els trastorns intestinals, com ara la malaltia de Crohn o la celiaquia, o d&#8217;altres com l&#8217;hipertiro\u00efdisme, la psoriasi, els tumors o les inflamacions cr\u00f2niques.<\/p>\n\n\n\n<p>Cal dir que, en certes condicions i segons quins trastorns, el cos reclama m\u00e9s \u00e0cid f\u00f2lic que no correspondria, com durant la gestaci\u00f3 o la lact\u00e0ncia, per exemple.<\/p>\n\n\n\n<p>La manca d&#8217;\u00e0cid f\u00f2lic \u00e9s manifesta m\u00e9s sovint en gent gran, que tenen una alimentaci\u00f3 incompleta, en els indigents i els alcoh\u00f2lics.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Com l&#8217;hem de tractar<\/strong><\/li><\/ul>\n\n\n\n<p>\u00c9s important de trobar la causa d&#8217;aquesta defici\u00e8ncia, que podria ser una mala absorci\u00f3 intestinal o els f\u00e0rmacs esmentats.<\/p>\n\n\n\n<p>Per regla general, ens hi pot ajudar for\u00e7a seguir una dieta adequada i eliminar el consum d&#8217;alcohol. Potser convindr\u00e0 que prenguem suplements d&#8217;\u00e0cid f\u00f2lic en forma de comprimits diaris durant uns quants mesos.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"800\" height=\"533\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/carne-asada.jpg\" alt=\"\" class=\"wp-image-26563\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/carne-asada.jpg 800w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/carne-asada-440x293.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/carne-asada-768x512.jpg 768w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Anemia_per_manca_de_ferro\"><\/span><strong>An\u00e8mia per manca de ferro<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>El ferro \u00e9s un component de l&#8217;hemoglobina (tal com d\u00e8iem, una prote\u00efna dels gl\u00f2buls vermells), que <a>t\u00e9 una funci\u00f3 clau perqu\u00e8 transporta l&#8217;oxigen a les c\u00e8l\u00b7lules<\/a>.<\/p>\n\n\n\n<p>Quan el cos no t\u00e9 prou reserves de ferro (perqu\u00e8 l&#8217;aparell digestiu no n&#8217;absorbeix prou o perqu\u00e8 en perdem en grans quantitats), es produeix poca hemoglobina i tamb\u00e9 menys gl\u00f2buls vermells, cosa que perjudica l&#8217;alimentaci\u00f3 correcta de les c\u00e8l\u00b7lules. Aquesta an\u00e8mia per d\u00e8ficit de ferro s&#8217;anomena an\u00e8mia ferrop\u00e8nica.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Que causa aquesta an\u00e8mia<\/strong><\/li><\/ul>\n\n\n\n<p>El ferro que ha de menester l&#8217;organisme \u00e9s variable. En tenen m\u00e9s necessitat els infants (perqu\u00e8 s\u00f3n en l&#8217;etapa de creixement), els esportistes (pel fet de consumir m\u00e9s energia), les embarassades i les dones que alleten (per l&#8217;esfor\u00e7 suplementari que comporta alimentar el fetus o el lactant).<\/p>\n\n\n\n<p>Trobem el ferro en la carn i el peix, i tamb\u00e9 en els cereals, el cacau, els llegums, la fruita i les verdures de fulla verda, per\u00f2 el ferro que contenen els aliments del segon grup costa m\u00e9s d&#8217;absorbir que el de la carn i el peix.<\/p>\n\n\n\n<p>La manca de ferro sol ser el resultat d&#8217;una alimentaci\u00f3 pobre en ferro o d&#8217;una mala absorci\u00f3. Passa sovint en els vegetarians estrictes o en els vegans i, novament, en la gent gran i altres persones que tenen una dieta defectuosa.<\/p>\n\n\n\n<p>Una altra causa del d\u00e8ficit de ferro \u00e9s la mala absorci\u00f3 digestiva. La poden causar la malaltia de Crohn, la celiaquia o una secreci\u00f3 insuficient d&#8217;\u00e0cid p\u00e8ptic a l&#8217;est\u00f3mac. Finalment, tamb\u00e9 en pot ser el motiu una hemorr\u00e0gia descontrolada.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Com l&#8217;hem de tractar<\/strong><\/li><\/ul>\n\n\n\n<p>El primer pas \u00e9s descartar-ne com a possible causa, per exemple, cap de les que hem esmentat abans. En qualsevol cas, normalment cal que el pacient prengui suplements que aportin ferro. Si se&#8217;n determina la causa, \u00e9s probable que fem desapar\u00e8ixer l&#8217;an\u00e8mia amb l&#8217;aportaci\u00f3 extra de suplements.<\/p>\n\n\n\n<p>No sempre \u00e9s f\u00e0cil d&#8217;absorbir l&#8217;aportaci\u00f3 complement\u00e0ria de ferro, i val m\u00e9s que aquests suplements continguin vitamina C i es prenguin amb la panxa buida. Malgrat tot, en certs casos causen irritaci\u00f3 a l&#8217;est\u00f3mac, per la qual cosa s&#8217;hauran d&#8217;ingerir durant els \u00e0pats. El ferro pot tenyir de negre la femta.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"800\" height=\"533\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/higado-crudo.jpg\" alt=\"\" class=\"wp-image-26565\" srcset=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/higado-crudo.jpg 800w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/higado-crudo-440x293.jpg 440w, https:\/\/www.mgc.es\/wp-content\/uploads\/2022\/07\/higado-crudo-768x512.jpg 768w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Anemia_per_manca_de_vitamina_B12\"><\/span><strong>An\u00e8mia per manca de vitamina B<sub>12<\/sub><\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>La vitamina B<sub>12<\/sub> t\u00e9, igual que l&#8217;\u00e0cid f\u00f2lic, un paper clau en la s\u00edntesi de l&#8217;ADN i, com en el seu cas, quan en manca, es redueix la producci\u00f3 de c\u00e8l\u00b7lules noves, fins i tot dels gl\u00f2buls vermells. Una altra funci\u00f3 d&#8217;aquesta vitamina \u00e9s mantenir les beines que embolcallen els nervis, formades b\u00e0sicament per una subst\u00e0ncia lipoide, la mielina. La mancan\u00e7a d&#8217;aquesta subst\u00e0ncia origina trastorns greus en la transmissi\u00f3 nerviosa.<\/p>\n\n\n\n<p>En resum, el d\u00e8ficit de vitamina B<sub>12<\/sub> fa minvar la producci\u00f3 de gl\u00f2buls vermells i perjudica de manera progressiva el sistema nervi\u00f3s.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Que causa l&#8217;an\u00e8mia per manca de vitamina B<sub>12<\/sub><\/strong><\/li><\/ul>\n\n\n\n<p>Trobem la vitamina B<sub>12<\/sub> a la carn i als menuts, principalment, i en proporci\u00f3 inferior en el peix, els ous i la llet. L&#8217;absorbeix l&#8217;ili, la darrera part de l&#8217;intest\u00ed prim. Cal que intervinguin en l&#8217;absorci\u00f3 diverses prote\u00efnes que segrega l&#8217;est\u00f3mac i el p\u00e0ncrees, en un proc\u00e9s for\u00e7a complex. La mala absorci\u00f3 pot ser per l&#8217;acci\u00f3 de bacteris o de par\u00e0sits a la zona que roben la vitamina al cos.<\/p>\n\n\n\n<p>Una altra causa possible d&#8217;aquesta an\u00e8mia \u00e9s una disfunci\u00f3 de les prote\u00efnes de l&#8217;est\u00f3mac, en primer terme, o de les pancre\u00e0tiques, que s\u00f3n necess\u00e0ries, tal com hem explicat m\u00e9s amunt, perqu\u00e8 l&#8217;absorci\u00f3 digestiva sigui correcta.<\/p>\n\n\n\n<p>Tamb\u00e9 es pot donar com a conseq\u00fc\u00e8ncia d&#8217;una alimentaci\u00f3 deficient, sense llet i ous per exemple, at\u00e8s que no trobem aquesta vitamina en les verdures.<\/p>\n\n\n\n<p>Ara b\u00e9, la causa m\u00e9s freq\u00fcent de totes pot derivar en an\u00e8mia perniciosa, que sol afectar la gent gran. Consisteix en un error en el sistema immunol\u00f2gic que acaba destruint les c\u00e8l\u00b7lules de l&#8217;est\u00f3mac responsables de produir la prote\u00efna m\u00e9s important per fer l&#8217;absorci\u00f3 a l&#8217;ili. Aquesta prote\u00efna s&#8217;anomena habitualment factor intr\u00ednsec. A llarg termini pot degenerar en un c\u00e0ncer a l&#8217;est\u00f3mac.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Com l&#8217;hem de tractar<\/strong><\/li><\/ul>\n\n\n\n<p>\u00c9s cabdal de trobar la causa del d\u00e8ficit. Si es tracta de bacteris o de par\u00e0sits a l&#8217;ili, convindria seguir un tractament antibi\u00f2tic (o fins i tot quir\u00fargic, per corregir una possible anomalia anat\u00f2mica).<\/p>\n\n\n\n<p>Sigui com vulgui, s&#8217;haur\u00e0 de prescriure un tractament amb injeccions intramusculars peri\u00f2diques de vitamina B<sub>12<\/sub>. En certs casos, potser cal que el pacient segueixi el tractament durant tota la vida.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Prevencio_de_lanemia\"><\/span><strong>Prevenci\u00f3 de l&#8217;an\u00e8mia<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Bona part de les an\u00e8mies s\u00f3n conseq\u00fc\u00e8ncia d&#8217;una alimentaci\u00f3 inadequada.
