Poden definir-se com la percepci\u00f3, sovint molesta o incomoda, que el pacient nota del batec card\u00edac. El <strong>Dr. Eduard Homs i Espinach<\/strong>, especialista en Cardiologia del Centre M\u00e8dic MGC , ens explica les causes d&#8217;aquests batecs forts del cor estant en rep\u00f2s i el seu tractament.<\/p>\n\n\n\n<!--more-->\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Que_son_les_palpitacions_cardiaques_o_batecs_forts_del_cor\"><\/span><strong>Qu\u00e8 s\u00f3n les palpitacions card\u00edaques o batecs forts del cor? <\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Les palpitacions, conegudes generalment com a batecs forts del cor, s\u00f3n un s\u00edmptoma bastant freq\u00fcent de consulta m\u00e8dica i estan definides com la percepci\u00f3, sovint molesta o incomoda, que el pacient nota del batec card\u00edac. El pacient pot notar com una tombarella que li fa el batec card\u00edac, o com una pausa. Habitualment s\u00f3n benignes i acostumen a ser produ\u00efdes per una s\u00e8rie de factors.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Per_que_el_cor_batega_mes_fort_i_quina_causa_aquestes_palpitacions\"><\/span><strong>Per qu\u00e8 el cor batega m\u00e9s fort i quina causa aquestes palpitacions?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Cal comprovar que no siguin fruit d&#8217;un ritme card\u00edac anormal i ens trobem enfront d&#8217;una arr\u00edtmia. El 90 per cent o m\u00e9s de les palpitacions tenen la seva causa en factors que anomenem excitants. \u00c9s a dir, tot all\u00f2 que accelera el ritme card\u00edac i la seva freq\u00fc\u00e8ncia: el tabac, el caf\u00e8, el te vermell i te negre, l&#8217;alcohol a dosis molt altes\u2026<br><\/p>\n\n\n\n<p>Quan aquests es consumeixen o quan es practica exercici d&#8217;alta intensitat, el ritme card\u00edac pot accelerar-se, produint la sensaci\u00f3 de les palpitacions, que es perceben tant en el coll com en la gola o en el pit. No obstant aix\u00f2, encara que acostumen a ser una conseq\u00fc\u00e8ncia normal del propi ritme card\u00edac, \u00e9s important analitzar els seus s\u00edmptomes per a descartar que es tracti d&#8217;una arr\u00edtmia. Per\u00f2, a m\u00e9s dels esmentats, quines altres causes trobem a les palpitacions de cor?<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Hipertiro\u00efdisme<\/h3>\n\n\n\n<p>L&#8217;hipertiro\u00efdisme apareix quan la gl\u00e0ndula tiroides produeix m\u00e9s hormona tiroidal del normal, que \u00e9s la que s&#8217;encarrega de regular el metabolisme, aix\u00ed com la temperatura o el rellotge biol\u00f2gic intern. Les persones m\u00e9s afectades s\u00f3n dones entre els 20 i els 40 anys. Encara que presenta multitud de s\u00edmptomes com a set constant, irritabilitat, problemes amb el son o sudoraci\u00f3 excessiva, els batecs irregulars del cor tamb\u00e9 solen ser presents.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Embar\u00e0s<\/h3>\n\n\n\n<p>Des del primer trimestre poden apar\u00e8ixer les palpitacions en el cor, en particular despr\u00e9s de fer algun esfor\u00e7 o quan s&#8217;est\u00e0 cap per amunt. L&#8217;explicaci\u00f3 en aquest cas es troba en qu\u00e8 durant la gestaci\u00f3 el volum sanguini augmenta per a proveir a la mare i al beb\u00e8. El cor treballa un 50% m\u00e9s que en situacions normals i es contreu amb major for\u00e7a, per la qual cosa pot donar lloc a taquic\u00e0rdia.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">An\u00e8mia<\/h3>\n\n\n\n<p>L&#8217;an\u00e8mia \u00e9s una afecci\u00f3 en la qual la concentraci\u00f3 d&#8217;hemoglobina en els gl\u00f2buls vermells \u00e9s baixa. Aquests s\u00f3n els encarregats de subministrar oxigen i el ferro que cont\u00e9 l&#8217;hemoglobina \u00e9s la que li d\u00f3na color a la sang. A m\u00e9s d&#8217;altres s\u00edmptomes com a feblesa i marejos, les palpitacions tamb\u00e9 solen ser presents.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Estr\u00e8s<\/h3>\n\n\n\n<p>Patir estr\u00e8s, especialment quan es prolonga en el temps i es *cronifica, suposa un augment dels nivells de cortisol en la sang. Quan aix\u00f2 succeeix, la pressi\u00f3 arterial tamb\u00e9 es veu augmentada i el cor pot bategar fora del seu ritme habitual.<\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter\"><img decoding=\"async\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2016\/12\/cardio-redu.jpg\" alt=\"cardio\" class=\"wp-image-2930\"\/><\/figure>\n<\/div>\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Son_perilloses_les_palpitacions\"><\/span><strong>S\u00f3n perilloses les palpitacions? <\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Habitualment aquests batecs forts no s\u00f3n perillosos. Hem de tenir en compte que les palpitacions que tenen un risc de complicaci\u00f3 s\u00f3n un percentatge molt petit i s\u00f3n les que es manifesten, per exemple, fent exercici f\u00edsic, en fer esfor\u00e7os, que es toleren molt malament, s&#8217;acompanyen de dolor tor\u00e0cic, esbufecs, i fins i tot de s\u00edncope. Per tant, la gravetat de les palpitacions ve associada, sobretot, a la toler\u00e0ncia de les palpitacions i despr\u00e9s, b\u00e0sicament, a si tenim un cor estructuralment sa o si t\u00e9 alguna alteraci\u00f3 morfol\u00f2gica. Les ar\u00edtmies en aquest cas sempre serien la conseq\u00fc\u00e8ncia de la cardiopatia estructural, no la causa.<\/p>\n\n\n\n<p>No obstant, la immensa majoria s\u00f3n benignes. S\u00f3n molt tolerades, s\u00f3n a\u00efllades i espor\u00e0diques. \u00c9s a dir, tenen un inici i un final gradual i es toleren molt b\u00e9. La freq\u00fc\u00e8ncia card\u00edaca, la taquicarditzaci\u00f3, de vegades, de les palpitacions, no \u00e9s molt alta. En definitiva, per sort, la majoria de vegades el pacient no s&#8217;ha de preocupar en aquest sentit.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Noto_els_batecs_del_cor_molt_forts_Quan_haig_dacudir_al_metge\"><\/span><strong>Noto els batecs del cor molt forts: Quan haig d&#8217;acudir al metge?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>En principi el pacient acudir\u00e0 al metge de cap\u00e7alera i li explicar\u00e0 els s\u00edmptomes: els batecs forts, fins i tot algunes punxades que no s\u00f3n palpitacions. En aquest cas el metge far\u00e0 un diagn\u00f2stic b\u00e0sic per determinar si cal derivar-lo al cardi\u00f2leg, que ser\u00e0 el que far\u00e0 un estudi ampliat per determinar el tractament. O si no, el pacient pot acudir directament al cardi\u00f2leg.<\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignleft\"><img decoding=\"async\" src=\"https:\/\/www.mgc.es\/wp-content\/uploads\/2016\/12\/palpitaciones1-e1481797383651-300x293.jpg\" alt=\"\" class=\"wp-image-2956\"\/><\/figure>\n<\/div>\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Com_es_diagnostiquen_les_palpitacions\"><\/span><strong>Com es diagnostiquen les palpitacions? <\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Ja que les palpitacions s\u00f3n una activitat el\u00e8ctrica del cor, l&#8217;eina principal de diagn\u00f2stic \u00e9s un electrocardiograma, que \u00e9s com una fotografia del cor. Si en aquesta prova surt alguna alteraci\u00f3 derivem el pacient directament al cardi\u00f2leg. Per\u00f2, tenint en compte que les palpitacions s\u00f3n un s\u00edmptoma que pot apar\u00e8ixer al llarg de tot el dia o la nit, necessitem una eina que mesuri el batec card\u00edac durant un temps continuat. Com ho fem? Amb un holter: un dispositiu que registra el batec card\u00edac de forma continuada, normalment durant 24 hores. Seria com una pel\u00b7l\u00edcula del cor. El pacient el porta posat i pot fer vida normal, excepte fer esport i dutxar-se. Si nota palpitacions durant aquest temps, ho apunta juntament amb l&#8217;hora i despr\u00e9s es fa una comparativa amb el que ha registrat el holter per veure si hi ha correlaci\u00f3.<\/p>\n\n\n\n<p>Com hem comentat abans, tamb\u00e9 \u00e9s important con\u00e8ixer si tenim el cor estructuralment sa i per aix\u00f2 fem una ecografia card\u00edaca acompanyant al holter. En resum: amb l&#8217;historial cl\u00ednic, l&#8217;exploraci\u00f3 f\u00edsica i l&#8217;electro hem de ser capa\u00e7os de determinar si les palpitacions s\u00f3n de risc alt o no tenen major import\u00e0ncia.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Com_es_tracten_les_palpitacions\"><\/span><strong>Com es tracten les palpitacions? <\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>La majoria de vegades no cal un tractament espec\u00edfic per aquests batecs forts al cor. Perqu\u00e8, si jo s\u00e9, per exemple, que hi ha gent que beu una galleda de caf\u00e8 al dia, aleshores, disminuint el consum de caf\u00e8, les palpitacions acostumen a desapar\u00e8ixer. Per tant, si sabem quins excitants s\u00f3n els que precipiten, fent una reducci\u00f3 d\u2019aquests excitants ja ho tenim resolt. Tot i aix\u00f2, hi ha casos en qu\u00e8 les palpitacions, sent benignes, poden interferir en la rutina di\u00e0ria. Llavors podem fer un tractament espec\u00edfic per disminuir el to en\u00e8rgic unit a un tractament d&#8217;ansiol\u00edtics. En el cas que les palpitacions siguin m\u00e9s importants, realitzarem altres tractaments adaptats a la gravetat i els requeriments espec\u00edfics.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Alguns_consells_per_a_reduir_aquestes_pulsacions_o_batecs_forts_de_cor\"><\/span>Alguns consells per a reduir aquestes pulsacions o batecs forts de cor<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Per a reduir les palpitacions del cor, el primer que s&#8217;ha de fer \u00e9s identificar quina \u00e9s la causa d&#8217;aquestes. Encara que com ja hem indicat no solen ser greus i sovint estan relacionades amb subst\u00e0ncies excitants, la qual cosa s\u00ed que podem fer \u00e9s introduir una s\u00e8rie d&#8217;h\u00e0bits saludables que redueixin la probabilitat de la seva aparici\u00f3. En el cas que es deguin a an\u00e8mia, la tiroides o un altre tipus de patologies, \u00e9s necessari consultar amb un especialista.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Practica exercici amb regularitat<\/h3>\n\n\n\n<p>No \u00e9s necessari c\u00f3rrer una marat\u00f3 ni apuntar-se a un gimn\u00e0s per a aixecar pesos. Del que es tracta \u00e9s de trobar alguna activitat que ens permet desconnectar de la nostra rutina i dels nostres pensaments. A m\u00e9s, quan l&#8217;exercici t\u00e9 una certa intensitat, s&#8217;aconsegueix descarregar adrenalina, la qual cosa ser\u00e0 molt efectiva per a reduir els nivells d&#8217;estr\u00e8s i, per tant, el risc d&#8217;aparici\u00f3 de palpitacions.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Medita<\/h3>\n\n\n\n<p>La meditaci\u00f3 s&#8217;ha posat de moda i \u00e9s que els seus beneficis per a l&#8217;organisme s\u00f3n indubtables. Deixar que els pensaments circulin sense interferir ens ajuda a distanciar-nos dels nostres problemes. A m\u00e9s, redueix el ritme card\u00edac i ens ajuda a entrar en un estat de relaxaci\u00f3.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Redueix l&#8217;estr\u00e8s<\/h3>\n\n\n\n<p>Encara que resulta m\u00e9s f\u00e0cil dir-ho que fer-ho, \u00e9s convenient aprendre a relativitzar els nostres problemes i a viure en el moment present. Aix\u00f2 \u00e9s, ni recordant el passat ni avan\u00e7ant-nos al futur.<\/p>\n\n\n\n<p>Si hi ha persones o situacions que sabem que ens produeixen estr\u00e8s, \u00e9s convenient allunyar-se de tots dos. I l&#8217;estr\u00e8s es deu al treball o a circumst\u00e0ncies familiars, la cerca d&#8217;ajuda professional per a aprendre a gestionar les emocions tamb\u00e9 pot ser efica\u00e7.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Evita els estimulants<\/h3>\n\n\n\n<p>Prendre&#8217;s un caf\u00e8 al mat\u00ed pot ajudar-te a despertar i a enfrontar-te al dia amb energia. No obstant aix\u00f2, abusar d&#8217;aquest i altres subst\u00e0ncies com el te o les begudes energ\u00e8tiques aconsegueix un efecte contrari.
